There needs to be an outside agency that investigates these incidents by the police instead of the police investigating themselves. There are govt agencies that are in place to investigate nursing homes and hospitals if there are allegations of abuse/neglect, for example.

Like an independent Ombudsman (that is what we have in Ireland). Our system is far from perfect but this structure helps maintain accountability!

Right. For example, when there's an abuse or neglect allegation in a nursing home, an ombudsman can be assigned to investigate, which can be in tandem with adult protective services.
