When writing YAML, you may know that the `---` is used to separate documents. If that's the case, why do I see so many examples with a `---` at the top of the file? Is this a huge cargo cult or something else I'm missing?

Not just hashes. Virtually any type will dump with `---` at the start. AFAIK, it's part of the spec, and anything that doesn't begin with that separator is an ill formed document that is coincidentally accepted by many parsers
