because tid can change?
Why would a pkey on a table which in which the pkey never changes have bloat? Is the pkey written to every time a new tuple is written to the main table, even if the pkey itself didn't change? (I would have expected a min txid or something)

If it's PG, then updating the row will re-insert the row into every index unless some specific conditions are met to allow a Heap Only Tuple update, like there being a free slot in the same page as the existing tuple. I've never had much success getting benefit from HOT.
