Our standard instances are not on containerized/shared infrastructure, you get the whole VPS (if I remember correctly). I.e. you have dedicated physical CPU(s) somewhere in the world. There is still the host AWS hypervisor, but I think that’s less prone to noise neighbors.
On heroku your max concurrent connections are tied to your plan, so IO is much less noisy neighbor prone than web servers. At least I've never experienced inconsistent IO times on the DB in many years (web server is another story)

In general though, doing more work in the DB is usually better. I don’t think I’ve ever found a case where pulling calculations back into the app layer made things faster.
