Formatting a large number with commas makes the whole decimal discrepancy between countries that use a period and the ones using a comma for decimal separation even worse and makes reading for the latter ones harder - IMHO!

I suspect you'll get better screen reader results from spaces instead of underscores (which doesn't really exist as a number format outside of programming code). US English-localized screen readers should be able to handle the commas, but other locales might read it differently.

I’ve just read this thread now so long after it was tweeted that you might not still be looking for answers. My answer is that for screen reader accessibility you shouldn’t mess with standard presentation of numbers, because it will blow any chance to make them read correctly.

My personal preference would be no separators at all, because then I can’t be confused by commas or periods, and I can force my screen reader to read them in any way I like. Of course, that’s just me and my screen reader. It would probably be bad for others.
