What I do is: - Take a step back from the chaos and decide "What is the one thing that is most valuable (or that needs my attention specifically)?" If there's more than one thing I pick the most important. - Do nothing else until you've finished this. - Repeat the process.

I guess you could do that 80% of the time, and then pick a day -- for example, Wednesday -- to resolve "What [you] would feel guilty about if [you] didn't help".

To be honest, I bet your process is fine, and that what you really need is a long holiday.

I think the challenge of this is when you block others. I'd love to just focus on a specific task and ignore emails/Twitter/GitHub sometimes, but what if 5 minutes here and there from me could unblock a lot of parallel efforts? Things become fuzzy.

Yeah, this is a problem unique to core/opensource developers. Probably two problems actually: too much you could help with, and the worry about ignoring something for too long... I'm not sure how to prioritise but remember you have finite time and (technically) set working hours.
