Video from Ukrainian military, said to be from Kherson: Abandoned Russian kit, even a full uniform laid out on the ground. A possible explanation for this is that Russian soldiers are changing into civilian clothing and deserting, as has happened before in Ukraine.

At the moment, it doesn't make sense to say Ukraine's "counter-offensive has failed." The counter-offensive started weeks ago, with Ukrainian sources loudly announcing the coming operation in an effort to tempt Russian forces into west-bank Kherson.
