The only entitlement under Medicaid is an institution (including nursing homes). Home and community-based services aren't covered under Medicaid unless you have a waiver, which can take over a decade to get (even though HCS/HCBS is cheaper than institutions).
This is for home health aides, they are paid by agencies and the agencies bill Medicaid. Adult day cares and transportation to/from work the same way. Half the seniors in Brooklyn have this kind of health care, and they don't pay a dime for it. Why don't people w disabilities?!

I don't have any professional policy experience with MediCARE, just Medicaid. It's possible they are using both? In any case the system is poorly designed, agreed.
