The only entitlement under Medicaid is an institution (including nursing homes). Home and community-based services aren't covered under Medicaid unless you have a waiver, which can take over a decade to get (even though HCS/HCBS is cheaper than institutions).
This is for home health aides, they are paid by agencies and the agencies bill Medicaid. Adult day cares and transportation to/from work the same way. Half the seniors in Brooklyn have this kind of health care, and they don't pay a dime for it. Why don't people w disabilities?!
Part of why many disabled people react negatively to this is because the current national trend is to outsource LTSS to for-profit companies under a model called "managed care." Kansas was the first state to do this and it's been disastrous.
For example, say you have a worker come to your home to cook for you 2x a day. Managed care corporations decide you should be trained to do it yourself, authorize 20 visits. But you may never be able to operate a stove safely on your own, so people get injured or neglected.
