Or a general, overarching premise, but no concept of the little moments that must be accumulated to create it. Or a feeling of coolness taken from elsewhere, but no thought about how it was earned, or how it can be earned again.

YES. I've seen all of these, especially the last. Or the idea that backstory and lore is story, so all their effort is placed there in the expectation that a satisfying and engaging story will just appear on the other side.
