Are there any narrative games out there with a difficulty scale that correlates not to something like combat or action, but to the level of drama and social conflict you have to navigate? Where casual means everyone's easy-breezy and on deathmarch you're walking on eggshells?

Pathologic and Pathologic 2 both do this! You can become hated in different areas of the town depending on how you treat others - socially as well as like, medically.

Although there is plenty of action involved as well, so maybe not quite what you were after?

Yeah, I'm looking for a choice-based narrative non-action game (or at least with action being secondary to narrative) where the difficulty pertains specifically to how high-strung people are and how much drama occurs because of it XD

Ooh, I get it! You know thinking about it, the only game where I’ve seen this as a difficulty slider option is Civ. There’s a bit of it in Frostpunk as well but it’s on more of a societal level than interpersonal.
