Are there any narrative games out there with a difficulty scale that correlates not to something like combat or action, but to the level of drama and social conflict you have to navigate? Where casual means everyone's easy-breezy and on deathmarch you're walking on eggshells?

Yes its done usually by changing skill check costs or negative reputation penalties, I remember ME3 had a 'Story' difficulty rather than 'easy' that worked like this, it lowered the amount of paragon renegade points you needed to unlock certain dialogue options, good idea imo.

It would depend on what you scale, if youre scaling a mechanic, its done already in ME3 (paragon/renegade), Deadfire (character traits), Outer Worlds (faction reputation affected y scaling). If you mean changing the content, like dialogue, that sounds like a huge amount of work.
