i always struggle with middles, or how to bridge together important events between the beginning and end. i often find myself going, "okay, i want the characters to do A, B, and C in the middle, but how do they arrive at those points? and what chapters should they happen in?"
Usually when I struggle with this I find that one of the things I want to include in particular is throwing me off; it either needs changing, moving or removing. I play around by attributing the event to different characters, contexts, etc to see what I get.

What I’ve done for mine is to rewrite my outline, pausing after each beat to make a note on what every relevant character thinks about what happened, how they feel, what they might be inclined to do. This can lend itself to new, unanticipated ideas.

Sometimes those new ideas help me bridge the gap to the scene I want; sometimes they show me that it needs to go in a new direction. Takes time, but worth it — I now have an outline which makes sense for all my characters with their specific personalities, values and motivations.
