This is something I see in my ER reporting too (and hopefully can report on when we have a bigger data set) — facility fees vary significantly depending on which insurance plan you have. Bigger plans can demand lower prices.

The cause is the payor fragmentation, not whims of the provider. Blue Cross can twist the arm of a provider to reimburse an MRI less than the operational cost to provide it. Provider has to make up that money - so they charge others more.
