These are very likely insurers paying significantly different amounts to use the exact same MRI machine at the exact same hospital. ¯\_(ツ)_/¯

This is something I see in my ER reporting too (and hopefully can report on when we have a bigger data set) — facility fees vary significantly depending on which insurance plan you have. Bigger plans can demand lower prices.
