Lack of insurers in rural areas pre-dates ACA. "Highly concentrated markets" (one carrier had 90%+ of policies) common. Blaming ACA = false.

IF CSR goes away in 2017 agreement goes poof. If carriers price in no CSR for '18 they'll stick around even without CSR
