The most convincing correlate (see 'The Spirit Level') is relative inequality when comparing US to other rich nations and US states to each other across a wide range of indicators of well being. As long as tackling inequality remains a taboo in the US, the slide will continue.

I think a result of poor educational methodologies used to teach literacy since the 1970s and the promotion of a high carb diet. People are dying of insulin resistance related diseases and they don’t have the ability to critically analyse diet research.

isn't that a function of the US diversity? If you compare life expectancy compared to etnic origin isn't US borns higher? I read that Swedish-Americans have longer life expectancy than native Swedes.

I hope there’ll be some closer research into the numbers than simply portraying them as such. Disparities in nation-wide life-expectancy, especially of a country as large as the US, say very little. I can think of several possible explanations off the top of my head.

It's possible. It also could relate to the very low level of public expenditure per capita. And, undoubtedly, a great many other factors.
