The employer-based insurance system is a rare thing that health policy wonks on the left and right agree is bad. Attempts to decouple the two, from figures as different as Jonathan Gruber and Paul Ryan, keep failing because Americans tend to get scared of big health care changes.

Sanders path to ending employer-linked coverage is single payer. Others want to end the tax exclusion for employer plans, the foundation of this quirk, and replace it with something more universal. Lots of ideas. But those who favor the status quo are very good at scaring voters.

Medicare has high copays. If you’re under 65, you don’t understand what a sham that argument is. The NHS system of pre-Thatcher days is the best. You get your money’s worth with taxes that are centred on the rich.
