(1/2) I don't think the main problem is at the manufacturing stage; rather, it's at the importation stage. I have gone to Canada, gone into Pharmaprix (Canada's equivalent of a CVS), and bought insulin OTC for about 1/10th the US price.

Agreed. I said elsewhere the (economic) problem for a low-cost pharmacy to solve is quality control of manufacturers and customer acquisition. Manufacturers of generic drugs are common in India & China; the ones distributed to US pharmacies and hospitals are concentrated.
