Yap, getting confused or surprised is pretty much the standart reaction but I've never seen someone actually getting offended.
Never insulted, but multiple times in Europe i've tipped 20% and the waiter is confused, or thinks that I am confused and left too much money. The emotion wasn't close to offended or upset at any time..

Yeah in Europe we don't have tipping wages like the US, so we don't need tips to make up for our lack of income So a 20% tip is actually quite a lot and is usually only given if it's a large or irregular order, hence obvious confusion
