    The report I saw this morning was that there are about 21,000 cases, and about 10% of those are in serious condition in the hospital. There have been total recovered patients (727) than dead (427).

    Back to the original tweet, though - people seem to be really playing up OMG LOTS OF INFECTIONS! But it's not JUST about how many people have it, it's also about how sick people get WHEN they have it. And right now, people don't seem to be getting that sick.

    Unfortunately, there are always some people who get very sick when they get a coronavirus. They have co-morbidities; age, or COPD, long-time smokers, asthma, compromised immune systems. It can do a serious number on them–even lead to death. It happens. It sucks. It's not /odd/.

    Lots of mild cases of sniffles, coughs, aches, pains–sounds like what a lot of people erroneously call the flu, right? Seasonal cold? Yeah, that's because there are four endemic coronaviruses that just swim around the population.

    Literally everyone I know in public health has said, from the very beginning, that there are going to be many more cases we don't know about. In this case, that's a good thing-lots of mild cases means that this is…a coronavirus. They're everywhere.

    Look like the German cluster of may not have been evidence for non-symptomatic transmission after all []. The authors didn't even chat with the index case to verify her *alleged* absence of symptoms... So how did they draw their conclusions? 1/2

    Well, the good news is, since we know it's much much less likely that there is asymptomatic spread, we can drop the travel ban and quarantines! ...yeah. Sigh.

    TLDR? The German letter re: asymptomatic transmission of did not actually confirm the Index Case was asymptomatic before making that claim. She WAS symptomatic the entire time she was in Germany. It was a normal, unfortunate transmission path.

    Asymptomatic transmission is one of the biggest things that's been used to justify draconian travel bans and quarantines. Mistakes are going to happen when everything is moving this fast, but we HAVE to get better at making lack of clarity/certainty clear in publications!

    So maybe we need to talk about being really careful to note uncertainties like "we haven't talked to the index patient" when publishing things that have major policy implications.

    I believe it was Friday that I was chatting with a few folks about problems I was seeing with the German cluster letter. I suggested that maybe Index Case thought she had jet lag; floated the idea she felt crappy & picked up some good OTC meds from a pharmacy.

    Also, does this mean will be retracting... a letter? Has that ever happened? It'd be the height of irresponsible to leave that letter online; it's fueled paranoia, conspiracy, and two national travel bans and quarantine. (It needs to be retracted.)
