it's unlikely they're actually using your advice in the first place. they'll just use you for show: "we need to implement this economic policy because, unlike the one my fellow politboro member wants, I consulted with the help of expert economists from [us university]"

Of course. But that's true of most everyone who hires an economist for advice, so its not a good reason to reject any particular consulting client.

even if your advice was used to, say, starve a couple million people to death, would that really be a good reason to say no? how is that quandary different than that of any other type of consultant? i engineer roads for govs that use them for ethnic cleansing all the time
