He gets excited when he thinks about how great he is at the same time he is giving a speech. I've seen it before. He has a hard time breathing through his nose and speaking.

It might be congestive heart failure. Labored breathing is a symptom, and with all that junk food he eats, his arteries are probably clogged to the max. Too bad we can't demand that he undergo a physical (and mental!) examination.
