    Over discussions this evening with 3 friends from Sciences Po, I am asked why The Lancet seems to have been reluctant to ask questions about the widespread influence of the Gates Foundation in global health. It’s a good question and one that I need to reflect upon.

        We must remember: The comparison isn’t Gates’s impact vs. nothing, it’s vs. what Gates could have done instead e.g. organizing to build power to build representative global institutions capable financing, implementation, and accountability for R&D and healthcare delivery.

        Corporate philanthropy (CP) and the sphere of influence (SI). In weak economies with fragile governance who is going to check CP and SI. @May be a topic for new series on CP and SI (benefits vs harm).

        Many see this as a shift of power from nation states to big tech. Philanthropy from the proceeds of platform capitalism, as in the Victorian era. Extends to the fairness of the work sending wealth to the apex (eg Carnegie dilemma) & whether tax would yield similar impacts.

        When will those who are bottom of pyramid be heard by philanthropy or governments? When will a fair wage financial system be set up that people won't need philanthropy or subsidy by government?

        Indeed. The very definitions of global health, and institutional responses- govts, agencies, universities- have been distorted. Accountability, democratic control, and equality become secondary. BMGF or any other similar foundation for that matter

        I’m surprised to read that this seems to be a new topic to @TheLancet. It has been discussed/debated in the sphere of academia for years. Another question is whether the editorial team has/would be(en) biased against such topics if submitted to the journal.
