The question is whether this provides an accurate shape in difficult cases, such as when the curvature passes through the offset and thus generates a cusp.

Right. Now that you mention it, I vaguely remember having a bunch of code that intersected the bezier curves to find cusps, so perhaps it's not quite as easy as I was remembering. :)
