"Do not congregate in chokepoints because it prevents people from moving through them" is not a complicated principle and I do not understand why people so commonly fail to follow it.
But spatial empathy is basically below the threshold of something that evolution would select for.

I do my small part to select for it by deliberately inconveniencing people when they fail to exhibit it.

And I don't think "parts of their brains don't work properly" is either a necessary or helpful framing: avoiding bad behaviour isn't some intrinsic feature of humanity. Morality is learned, and people have a responsibility to learn it.
