Most of the time. I've usually got some sort of proof-of-concept to give me enough confidence at the time I submit the talk, and a fair idea of what the best- and worst-case scenarios are for the presentation I'll finish writing the night before...
I'm at a bit of an advantage, though: I speak often, so I know that there's almost always the possibility of falling back to giving a talk I've given before, if I'm not ready. I'd rather not to something that's not what I promised, but people usually don't mind.
