'Religious liberty' includes the freedom of communities to impose their own moral judgments & social norms. 'Equal protection under the law' means everyone must be immune to those judgments & norms. There is no credible way to reconcile these conflicting principles. Never was.

In general it means people who share strong opinions with a large group of people and who are willing to suffer serious harms to in order to act on those opinions. This is why you get an "exemption," because enforcing law in this situation will often do more harm than good.

If you don't have a large group, no motive for exemption, because harm to law will likely be worse than harm to those few people. If you aren't willing to suffer for your opinions, no motive for exemption, since enforcing law will not cause any special harm.
