It would never happen. The risk is almost 0% (accounting for rogue insiders/ terrible accident) because both sides know it would mean the literal extinction of every human on earth.
I highly recommend Daniel Ellsberg's book 'The Doomsday Machine', for anyone who thinks the risk is low.
