If my workload does 1 delete and 1M inserts is this going to trigger a full vacuum, scanning all secondary indexes, when I really just want visibility map bits to be set?

You could set "vacuum_index_cleanup = off" on the table if you knew it had this workload. Otherwise, yes.
Unless I miss something that workload would *not* trigger at least btree index scans for vacuum. If there's no dead tuples, we don't scan those (unless close to a wraparound).
