Yeah, that's why I wound up having to use futex directly in https://bugzil.la/970676 — we believed it was possible for tgkill() to succeed but not call the handler if the target thread was exiting, & sem_timedwait doesn't have a monotonic variant (except on QNX).
(glibc doesn't have that problem: IIRC it uses its own list of threads, and takes an internal lock to exclude concurrent thread creation/exiting.)
