The count is usually based on the use type and a % of tot ocupancy count. Not on each other. But women sometimes have higher %.

2/ my guess is that someone installed 1 more than showed on plan (and required) and upon fail insp that cheaper/faster than refile amendment

Those are waterless urinals. They don't work well and stink. That one got abandoned in place for some reason.
