How much is due to bad healthcare vs bad health choices? Obesity shortens lives and is not cheap, and there's a lot in the US.

And that's just one issue, people also aren't very good with preventative care, even when they have insurance that covers it.

my personal guesstimate is, it's about half a problem with the healthcare industry, & half a problem with the food industry
