@paulg $6.1B CATIA A380 debacle comes to mind - that's a real bug: http://calleam.com/WTPF/?p=4700 First few couldn't be type-certified!

@DubDublin Was the older version of the software wrong though or were they just different? Sounds like the latter. -
@paulg@DubDublin Not sure, but V4 and v5 computed wire bend radii differently. Mixing versions apparently made it worse. -
@paulg FWIW, I'd call it a bug, based on http://www.cadalyst.com/cad/product-design/what-grounded-airbus-a380-1090 …, but YMMV. -
@DubDublin That 404s.

@paulg was it@NASA_Hubble that failed to factor in the paint on the mirror's milling machine? -
@sneakin@paulg@NASA_Hubble that doesn't sound like a bug. That was more like a missing test. -
@manuelhe@sneakin@paulg@NASA_Hubble That is much more a corporate culture problem https://en.wikipedia.org/wiki/Hubble_Space_Telescope#Origin_of_the_problem … cf Feynman critics.

@paulg A bug in an electonic CAD tool made a chip dead on arrival, by confusing greater than with lesser than. Despite all the testing. -
@paulg I can think of a few cases like@alain94040's. DoA isn't so bad. The worst are subtle issues that aren't caught until after release.

@paulg not exactly CAD, the Sleipner A offshore platform sank as a result of insufficient gridding in an FEM model. - 1 more reply

You can hear many such stories from any CNC operator. Where public safety is at stake, CAD output is typically verified by humans, with different degrees of approximation, with particular attention to frequent sources of error (meshing and such).
