Just software would help. You would be surprised at the amount of manual processes at legacy banks. Things like this happen all the time, albeit for much less financial and headline risk

Yes, much much harder. I mean ideally the software can be smart enough not to even require the crazy workarounds that Matt Levine details in the first place, but even if it did, the UI/UX and the approval flows are neither intuitive nor effective.

You should write something explaining why. People who are interested in the workings of complicated systems but aren't domain experts in this field (i.e. HN readers) would love to hear more about how such mistakes happen, and how they can be avoided.
