In the teenage world you win by perfect performance on easy tests (wearing just the right clothes, answering multiple-choice questions correctly), while in the adult world you win by adequate performance on hard tests.

I think that's due to individual human weakness. Telling someone who worked hard on something that it's no good is painful. Maintaining accurate, non-inflating grades requires serious discipline.
One difference between school and the "real world" is that in the latter you're ultimately judged either by mother nature or by customers, and neither of those feel any pain in telling you that you did a bad job.

If you define "real world" as startups, maybe. In corporate environments, many people are insulated from customers and the only people who can provide feedback are colleagues... who for a variety of reasons may not feel comfortable or even safe doing so.

Even in big companies the ultimate judges are customers and mother nature. It's true that layers of bureaucracy can often mitigate the shock though.
