Perhaps one reason Italian Covid-19 deaths are understated is because individual doctors don't want to make people feel guilty for their whole lives for infecting grandma with it. So they just write "cardiac arrest" without inquiring further.

The families of those who die at home may be reluctant to report or get an official test for fear of forced quarantine.
It’s more simple: we aren’t testing the deceased for Covid-19. If the patient dies before they can give him a test, he won’t be record as a covid-19 death
