Perhaps one reason Italian Covid-19 deaths are understated is because individual doctors don't want to make people feel guilty for their whole lives for infecting grandma with it. So they just write "cardiac arrest" without inquiring further.

Not sure we can extrapolate this to all of Italy as the author suggests. If Nembro was particularly hard hit, we might expect to see less accurate figures here than in other regions.

The "cleanest" and only actionable indicator is the number of ICU admissions. It can be compared across countries (as opposed to testing and causes of death) and it's what matter ultimately, how fast a health system is heading towards collapse.
