Usually the latter, but sometimes you can overpay for top execs who look good on paper and negotiate for a high salary, and then don't deliver.

I was in a startup, where the pressure from investors to grow faster almost bankrupted the company. The reason was: burn money faster to grow faster. This did not work. Obviously.
