I usually think the first paragraph or two, often as I'm walking somewhere. I come home and write them down, and keep writing. I may have a list of 5 things I want to remember to say, but never more. It can take from a couple hours to a couple months, but typically a week.

how often do your external editors/proofreaders give an insight that makes you rethink a whole piece? Any examples?

On at least three occasions that I can remember, they've caused me to kill whole essays. Many other times I've done major rewriting after someone pointed out a mistake.
