In my experience launching new products in a very large company, employee trust and empowerment is a huge factor. Also need sign off procedures that get out of the way of innovations and a flat org structure that gets new innovations to executive decision making level faster.
Last line could be replaced by: -new idea, back to line 1! -someone else did it lets just buy them -it was a bad idea because we couldn’t do it and we then ignore any success by someone who did it until its too late -what were we working on again?

HA! Exactly what I was going to add. That or three years later a watered down new division is added that flops within 2 years.
A few years later they'll restart at step #1 and make all of the same mistakes because everyone who was around for the first iteration moved on by now.
