I’m choosing to believe that that the real story is that the ICBMs came with default launch codes printed on them, just like a wireless router, and no one’s niece or nephew had the security clearance to come in and set up new, secure launch codes.

I know that example is for educational purposes but launch codes had a completely different context than passwords They were another small step in a very long chain of execution of launching ICB's
