One sign of a startup idea is the word "shouldn't." If you find yourself saying "users shouldn't have to do x" or "we shouldn't have had to build y," it's often a sign of a missing startup. And if you're the one saying "shouldn't," that you would be a good person to start it.

Also, I shouldn't have to travel 50 miles to go see a patient. I promise there are at least 3 nurses with equal or better skill within a 10 mile radius of that patient. Time wasted driving = less time to care
