One way to tell when a design has failed is when you have to add notices telling people how to use it. Push the start button to start. Click here to adjust whatever.

(The absence of such notices doesn't mean a design is good, though. If autocomplete shows lots of people have to search to figure out how to use something, that counts as a notice.)

Out of this mindset emerges a culture of incrementality, i.e. new UX always being a copy/slight alteration of familiar UX. World of UI/UX design is monotonic partly because of that.
