Numbers 2 and 3 seem nearly identical. But what is the underlying factor that makes America so bad in this respect? And is it something other countries will start to see to, or is it unique to America?

Yes, 2 & 3 closely correlated, but not always identical. People can overweight but still get enough exercise. Or eat a high-quality diet, but simply eat too much of it (and the opposite - not overweight but not a nutritious diet). But correct that they often go hand-in-hand (1)
