If their employers aren't Japanese, how can they live and work remote there? Under a tourist visa and getting in-and-out of the country every n months? Or is there a special visa for these cases? Very interested in this kind of information!

Employ under a local subsidiary of the company, joining a contracting firm and then having the company purchase one’s services through it, working holiday visas for countries which support it, etc. Also quite a bit of “How likely are they to find out?” in my experience.

You'll be covered by either your employer or the national scheme, compulsorily. Experience of accessing it in English might be a little frustrating.
