He's including things like stock. Also, it's *mostly* only the big 5-or-so that can pay so highly reliably. "Mostly" meaning, like, certain small boutique companies can bill big companies enough to make that happen, e.g. infrastructure or consulting companies.

Only the few people earning this much need to add huge value. The key is scale. The software they’re writing must make a correspondingly huge impact on savings or earnings.
