Ogaki City Hall did this with a pre-filled form and a pre-addressed stamped envelope. "We need bank info and signature in < 6 months."

When I filed for it after 6.5 months I got a blisteringly polite form letter saying, effectively, "IF YOU COULDN'T DO THAT IN SIX MONTHS..."
