Back in the day, I was compulsorily enrolled in Japan's national insurance scheme for self-employed people. The rates are dependent on income; I think it was on order of $600 a month (approximately the cap). Note that this offering has 30% co-insurance in most cases.

(i.e. If a hospital billed $100k then Japan's national scheme would pay $70k and I'd pay $30k.)

Important clarification would help — what happens to “major surgery” type of bills? Like, ¥10M? Are you bankrupt, or there’s an out of pocket max of some sort?

There's some sliding calculation for maximum co-insurance per month based on one's income (and whether one is over 70/etc). People do buy supplemental insurance for e.g. cancer because it is possible to run up material expenses under the Japanese system.
