Back in the day, I was compulsorily enrolled in Japan's national insurance scheme for self-employed people. The rates are dependent on income; I think it was on order of $600 a month (approximately the cap). Note that this offering has 30% co-insurance in most cases.

That is surprisingly (to me) ungenerous. Do most folks get 100% free from their employer or is that indicative of health insurance costs in Japan?

Most people are on employer-provided insurance, not the national insurance plan (covers principally retirees, unemployed, self-employed). EHI generally also has 30% co-insurance. Some companies may add additional benefits which decrease cash out-of-pocket for employees.

I do not recall the specifics of the EHI offering for any company I could licitly tell you about. It is my general impression that large companies would optimize for salarymen having minimal extraordinarily healthcare costs; I would be surprised if all companies could afford it.
